troubleshoot this FT issue

Rx is +1.50 -2.25 x 90 OU with 2.50 add no prescribed prism
fit FT28 poly
symptom is when patient looks down through the reading add he sees a an inverted triangle shape (image is wider at top and narrower at bottom)
distance vision is normal
when he covers one eye the problem goes away

What's your solution?

Move lenses closer if possible, and increase panto. ( and raise seg ht (if possible).

Due to the rx the lenses might be warped. Check the front curve horizontally and vertically.

I like Lensman's idea. A slightly large lens torqued into a metal frame? Causing a toric front and back surface?
