The thin layer of transitions has separated. My guess is that lenses are not a true Transitions product. I used a finished poly chromatic from a supplier and had similar failures within a year. Lab probably substituted with a non branded "Transitions" lens. Extreme heat can also be a causal factor. Ask patient if they leave eyewear in a vehicle or direct sunlight. I would reorder in Trivex or ask for proof true Transitions was used as failures like this are rarely found with Signature 8 product.

Re heat the ar coating doesn't seemed damaged so i ruled that out unless that is still possible (the customer lives in Spain and we have been having v high temperatures, I'm actually based in the UK)
The company i got them from is part of the Essilor group so I really hope they are genuine ( a transition card comes with the lenses) but nothing would surprise me these days
Im sending them back to the lab for a replacement hopefully free of charge

Anyone have issues with the new Polarized transitions not getting dark?
Is that the new generation Xtractive polorized?

There have been issues with the Transitions Xtractive Polarized that's why the release date was delayed.
I got a pair of Xtractive Polars a few months ago and they are very nice. They get pretty dark and are very comfortable. I have them in 7EYE wrap that I wear when I ride my Harley.
That picture reminds of the old Life RX photochromic lenses. I stop using any of them after six months because a very high percentage would delaminate and look exactly like this.
I think they were the only poly FT photochromic option at one point. Stay far away from any laminate based photochromics.
