    Quite a scholarly work! Great job.

    To dumb it down to my caveman level:
    1. Anisomyopia: Don't do anything design-wise. The BC increments are pretty stable in minus powers. The CTs are always minimized anyway.
    2. Anisohyperopia: Order equal BC and CT.

    Is that enough? Well, it minimizes unequal image sizes but doesn't correct for unequal image sizes. That should get you by, though, about 99% of the time.

    You bring up pediatric cases, and that's where it gets interesting. Anisohyperopia is the real clinical entity out there (way, way, way more than anisomyopia) and resultant anisometropic amblyopia. There is reason to believe that iseikonic lenses can help develop completely normal binocular vision, vs. just having the kiddo develop with reduced stereopsis and binocular function in general.

    There should be lens design programs that help. I know that's what Shaw does, and that's pretty cool, and it currently IS NOT the standard of care, but should be (and will be, some day).
