    Help Identifying this Progressive

    Hey, I need some help identifying this progressive lens that a patient brought in. They were lead to believe that it was an Essilor lens by their Optician at another office, but Essilor was unable to find it. I've looked through my Progressive Identifier cover-to-cover with no luck.
    Above the nasal watermark is: "D5M2"
    Below the nasal watermark is : "49"
    Below the temporal watermark is the add: "25"
    The watermarks are the typical circle watermarks.
