    Shift value for Computer/Reading

I have a script with the readings

    sp cyl Ax ADD INT
    +1.50 -0.75 100 +2.25 +1.50

does this mean a shift of +0.75
Thus making the sp +2.25 ?

Optical store is saying something else

No. Standard intermediate is 50%. The OD is requesting a 66% intermediate for your trifocals.

This is an extended reader not a trifocal.

Should the sphere be +2.25 or +3.00 leaving either +1.50 or +0.75 for the reading section?

The 66% rule still applies. Any competent optician will be able to transpose this for you. I’m assuming that you are not an optician?

You seem to be confusing the term 'shift' with 'add'. An extended reader with a +1.50sph(and the cyl x axis) and a shift of +0.75 would have an Rx of +1.50 at the fitting point and +0.75sph for the 'extended' part of the reader.
The Rx you have given is Distance, Reading add and Intermediate add so the rule lensmanmd is quoting to you applies.
