Prism billing question

Hello!

I have a patient that we needed to use slab off prism for and I'm wondering how to fill out the claim to Medicare for it.

Do you bill for the prism as normal (V2715) as well as the code for slab off (V2710) or do you just use the code for slab off?

Slab code only. If there is prescribed prism in addition to a slab, bill for both.
