    Confused Form for Patients to self submit to their insurance

    Our practice does not taking any vision plans for hardware. I would like to know if anyone has a form they have put together that can be given to a patient with information that would help them submit for reimbursement from their insurance. Or how does your practice provide information to patients so they can self submit.

    You are looking for what is called a "Super Bill." If you Google "health insurance super bill" you will fin all the information that you need.

    I take it as a big compliment when my customers opt to buy from me instead of their insurance provider, so I take it a step further and submit out of network on their behalf. Giving them all the info they need is helpful, but it's still challenging for some to do it right (or at all), and the insurers don't always make access to the reimbursement forms easy. So for these valuable customers I provide OON filing service. Here is my collection of forms:
