Importer use UPS customs broker service

I'm going to import some stock prescription lenses from my oversea's supplier and they are using UPS worldwide experdite to ship the lenses.
Does anybody ever use their customs broker service? I notice there are a couple of fees listed on their website but I'm not sure which fees will be applied to my shipment. Does anybody know what kind of fees I need to pay and how much it's going to be.

Here are the fees they listed
