What does everyone bill or what should I be separating out for the Crizal UV charge for Eyemed and VSP? I thought traditionally it had been 20.00 but who knows if this was correct. Thanks!

With VSP we bill an extra $10 if their insurance doesn't cover UV. With Eyemed it's generally either covered, is $15.00, or else we give a 20% discount on our price.
The problem I have with the Crizal UVs is the if it's anything other than plastic, it's already 100% UV blocking. I shouldn't get the charge back for it. It's the principle of the thing.

Yes, that's a misunderstanding.
Crizal UV actually doesn't block UV transmission through the lens.
It reduces UV reflectivity on the backside of the lens.
AR coats prior to the "UV" line actually exacerbated backside UV reflectivity back into the eye and perioptical region. The "UV" tech ratchets that level back down to what a non-AR lens would be.
You still need a UV-blocking lens material with a Crizal UV treatment in order to have "UV protection."
