Scott Harris Vintage VIN-17, its a 42, but close to what you're after perhaps.
Smilen Optical - Profiles series. I know they have one at a 41 eye. He'll let you buy one, of course volume helps. Basic entry level frames. 1-800-887-5675.

He's rough on his glasses, small children, pets, etc. Hoping for something that won't fall apart in a year.

In that case...go with Uilleann's recommendation! I sold that very frame in a +8.00 ou ft28 1.67, and it was a model of perfect decentration. CT was outstanding and the patient couldn't believe "how thin." I buy the profiles half a dozen or so at a time for the two or three patients that want them a year. I price them at $99. I have a guy who is particularly rough on them, he has averaged one a year, so maybe skip those for a rough patient.
Keep hearing great things about OVVO, just don't have room for them yet.
