I am thinking the North American Rx would be

O.S. -2.50 -0.75 x 057 Add +3.25 ou

Correct me if you are Polish and this is wrong. lol.

This is not a drug Rx, it is simply for a mechanical seeing aid.


If an examination indicates that corrective lenses are appropriate, the prescriber generally provides the patient with an eyewear prescription at the conclusion of the exam. In the United States, the FTC (Federal Trade Commission) requires eyewear prescribers to give each patient a copy of their prescription, immediately following an exam, even if the patient doesn't ask for a copy.

I had something similar come in a couple of years ago but it was Russian. My general rule for any RX is if I cannot read at 100% I don't fill it without verification. I would ask the prescribing office to verify and if that isn't an option I wouldn't fill it. I tell patients I am not a good speculator or guesser.
