Zyl semi-rimless, plastic on the bottom with the rimless portion up top, oval or half-moon shape. Any ideas? I could have found two dozen of these in 1989, can't find one now...

Tura\Titanflex has some
I went on EBay looking for an old "Peepers' or Formost M-225 but no joy, However if you check out "half eye frames" you will come up with a ton of stuff. I'm surprised that your customer has not already done so.

Must be ophthalmic, in case I hadn't made that clear.

Could have been May or any one of the many US frame manufacturers that have been relegated to the ophthalmic dust bin. It was a long time ago and I only have a few brain cell still functioning. All that youthful booze and drugs has taken it's toll. I'm sure that there is an anal retentive optician out there who still has a 1972 copy of Frames who could set us straight.
I do recall the "Peepers" and the "MT-225" as we sold a ton of them. They were a joy to glaze and did separate the men from the boys. They both had metal eye wire tops and you had to edge them rimless and bevel them by hand. Almost as much fun as fabricating a pair of cataract lenses in a Polymil mounting.
How about the ole TOP NOT 310
try ich berlin
