Add sphere to 50% of the Add Power, thats simple to get to the Intermediate. Never had it backfire on me, if you want to keep it simple thats it. Put the remaining add on the bottom of the lens in the reading segment and they will be very happy.

+1, generally. The patient may want to discuss this with their doctor, however; you can get a more accurate pair if they measure the distance from their eye to the monitor(s) in their workplace. But the 50% of add power formula works over 95% of the time, in my experience. The exceptions are unusual workspaces and persnickety patients such as engineers.

Alternatively, you could try a computer-specific lens. I've had great patient response to the cleverly-named Essilor Computer lens, but other companies do similar things.

Instruct the client to measure the work distances, the corneal plane or eyeglasses (add 14mm) to center of the monitor, and to the desktop. Determine that the add for the reading is for 40cm. Trial frame using these starting points: 80cm- 40% to 50% of the add, 65cm- 50% to 60%, 60cm or less- 55% to 65%. I use a ±.25 D flipper over the trial lenses to tweak. Use a multifocal to cover the desktop/40cm work distance.

If your trying for an all in one solution/SVNO (usually not recommended), balance the blur as needed, but make sure to trial frame so they can see the compromise in vision.
