    Help with prescription

I need to know what this RX means

    OD +2.75

    OS +2.25-1.00X180. READERS

    Drop 1.00

Is drop another reference for add, what does this rx mean? Is it a progressive or does it mean 1.00 drop for intermediate glasses or a computer lens possibly?

    thank you

Why not call the prescribing doctor and ask him/her?

If the patient is 41, yes it is.

Another interpretation is that the prescriber is concerned with vertical imbalance, and they want you to drop the OC 1mm from the frame's horizontal midline or 180 degree line. However, the OC position is frame dependent, and asking for a drop of 1mm might just be their way of saying "watch it". Don't guess, measure the reading depth and position the OC appropriately. As usual, call the prescriber if there are any doubts whatsoever.

If the Rx was prescribed as readers, then a reading add would serve no purpose if the Rx was intended for progressives. Drop could suggest reducing or 'dropping' the sphere power by 1.00D OU for an intermediate pair, or for a NVF office lens as OP postulated. NVFs are often ordered with the near Rx, and the add indicates the power degression or reduction to achieve the intermediate power. This is the closest synonym for 'drop' that I could think of, other than Robert's suggestion of dropping the OCs by 1mm, which is also a strange request.

Anyway, its all just speculation; only the prescribing doctor can clear this up.

Need some help calculating the intermediate Rx...
    Current Rx
    +2.00 +0.50 x 180 2.25 add
    +2.00 +0.50 x 161 2.25 add

pt wants intermediate/ near FT and says monitor is 24" away. Normally I would split the add but in this case do 1.00 or 1.25...my OD prescribed
    +3.50 +0.50 x 180 0.75 add
    +3.50 +0.50 161 0.75 add
I'm concerned that 1.50 is too strong for 24" working distance... any advice?

    24" (2.54 cm/1") = 61 cm

    1/0.61M = +1.6 D

I would never make it that strong, either, mervinek. I don't ever exceed +1.25 and I prefer to go with +0.75.

I don't think patients can measure. I don't think patients sit still. I don't think it's fun to walk around in a +1.50 add to get a cuppa joe.

It's just a bunch of trouble for nothing. Half the add is more than enough.

Having said that, there are some weird cases out there, occasionally.

Well that was my thinking too! I did discuss with the lab and feel like I will have a nice little chat with the patient before ordering anything so she knows exactly what she's getting. I feel like it's a bit strong, but she does have the ability to move the monitor and will have to tell her it not be great for anything beyond monitor. I feel like you can know the formulas all you want, but a good optometrist knows when to adjust it a bit. That really makes the difference between a good optometrist and one who is just an optometrist.
