Does anyone know of states, other than MN (MN Care Tax of 2%), that also charges a tax on eye exams/services and/or ophthalmic goods.

Ohio I believe charges tax on eyeglasses and goods. Pennsylvania charges tax on cleaning supplies, cases, accessories, and plano sunglasses.
The Minnesota tax is the chief funding mechanism for MinnesotaCare.
Florida checking in here. No tax on exams or prescription eyeglasses or contacts. Everything else gets the going tax rate of the local area.

Nevada slipped a repeal of a tax on ophthalmic goods through about a decade ago. Voters thought it was a great idea to get rid of a tax, what they failed to read in the small print of the question was that the full state sales tax of 7.725% would replace the 2% flood tax that was currently in place. Crafty legislation to say the least! But hey, at least exams aren't taxed- yet...
NJ, no tax on Rx , tax on planos and sundries. NH, 9% tax only on prepared meals. Rx glasses isn't a prepared meal, is it?

Can't keep voting for more taxes and then complain.
IL 2.25% on medical goods, no tax on services, 9.25% (in my area) on non prescription goods.

Florida = no tax on Rx ophthalmic products or exams
Mississippi = no tax on exams, but tax on all ophthalmic products (Rx and non-Rx)
No taxes on spectacles, contacts or exams in Maryland (Where taxes are actually our state mascot, not the Chesapeake blue crab like people think). Cleaning stuff and plano sun is taxed.
