You listed the three likely possibilities but we need more information.
What's his prescription, what lens design is he in, and what is the B measurement of the frame?
What's he trying to use it for? A computer screen straight ahead?
The complaint can result from all of the above that you stated. The complaint could also result from the person's requirement of a generous intermediate area within the lens, and sadly, the "intermediate" area within the lens is the smallest useful area within most multifocal designs. If the person's requirement is for a larger, or higher useful intermediate power, I would also suggest either a trifocal, or a multifocal "office" or "computer" or "room" design.

Not much to go on, but here is some food for thought. I have seen this happen and more often than not part of the problem was a significant change in add power. If the patients old pair were a +1.75 and now the new ones are +2.50 that could be the problem. The patient had adapted to what amounted to a very large intermediate because of the decreased add power, and they are used to the weaker rx.

I agree with eryn if they had a weaker Add. Pretty common and not well known amongst patients- bump the Add leads to squishing the mid. But this often leads to a second pair of office/computer lenses and they are so thrilled afterwards.
Plenty of varifocals are pretty weak when it comes to intermediate performance. Most of my patients who wear varifocals generally are also dispensed with a pair of occupational degressives, or 'indoor' PALs.
Perhaps you should consider alternatives for use with the computer. :)
