I'd check calibration of drill/diameter of holes being drilled. If they are too big, magnets can pop out of clip easier, and if they are too small it can stress the clip and cause breakage.

Is the magnet coming out of the Rx lens or Chemistrie layer?
As noted by vcom, it could be a hole size problem. If in the Rx, it could also be either a lack of glue or the wrong glue. The lab should only use MR290 glue for the Rx magnet.

Id check all of the above, However we discontinued the use of this product due to these same issues.

I have absolutely no problem (and I mean none) save for one individual that is rough as a cob. We do anywhere from 6-18 per week and have for the last year and a half, you can do the math on how many we have out there on clients glasses.
If mags are coming out of primary lenses, check hole depth and diameter. Do as Anna instructed: Do not scrimp on the glue! Use so much you have to clean excess off the lens. If you are having issues with layers coming apart at the bridge check hole size. Make sure you are widening the compression mounting prior to crimping the bridge. Work your way down the crimping tool's slots to insure you are inserting the bridge squarely and straight.
If you are having any other issues, go ahead and post them. Some of us here have done a lot of them and probably have a solution for you.
The only ones I have lost went through the washer and dryer, after doing a few hundred sets we have zero issues.
If your having lots of problems, its from whoever is doing them is doing a poor job.
I just asked the lab to send me some because a magnet fell out of the Rx. I looked up MR290 and it says it's a cyanoacrylate adhesive which sounds curiously similar to "superglue." Is it different? Can MR290 be used to glue a lens in a frame?
I believe we were the first Optiboarders to use them, and after a few thousand, we've had no problems.

I believe Pacer Industries which makes the MR-290 product is part of the Superglue Corp. which has several families of adhesives. Pacer makes their industrial glue and they have over 20 different adhesives designed for different materials each with different cure times, cure properties, etc. I know that early on Dennis researched and experimented with other glues (including common super glue) and he found this to be the best for gluing the magnet into the Rx. It does not expand and cause crazing/cracking nor does it contract and allow the magnet to fall out. We have had labs try to substitute cheaper glue and it has led to problems.
Chemistrie clip is a very nice clip if You know how to work it correctly! I buy from Eclip, clip house, chemistrie, air clip....and they all have their own fans. Sometimes, I have to mix them together for very custom made.......
