    Medicare Template

    We are not Medicare providers at my office, but we help the patients fill out the forms for reimbursement, but, lately they have all been getting rejected, does anyone have a template for filling out the forms? Or suggestions as to what we might be doing wrong? thanks in advance.

    Glad to see that I'm not the only one with Medicare problems. We do not accept any insurance, but we do fill out the Medicare form and send it in for the patient.
    Their HUGE problem is using modifiers. Not only do you have to use RT/LT if it is a NEW item, it needs a NU modifyer also. I just had a conversation with Medicare yesterday over the fact that they said I "left off" modifier KX (medically necessary)...told them I DID NOT leave it off. The patient knew it was a non covered service ( poly,trans,ar...whatever) and wanted it anyway. BUT THEY PAID THE PATIENT ANYWAY....everytime I get off the phone with them I just want to bang my head on the wall... a few weeks ago I pulled info from their website. When I called to ask them about it.. I was told that info was incorrect, where did I get it...when I told them from THEIR page they had no answer...grrrrrrrr send me a pm and I can send you a copy of my "mock up" form.

    Oddlly enough, of late Medicare (while still a pain in the A) has become the easiest of third parties to collect from amoung third party payers.

    We have just become approved for Medicare and I am attempting to set up their free ETI software but the ONLY thing I can't find is a fee schedule for the actual lenses (frame, I actually remember). Anyone have an idea where I can find it? We had a great one at a place I used to work at but they guard it like the crown jewels for some reason. :)

    go online to your local DME. Request vision codes. Easy breezy
