Could the vertex distance be adjusted to create the needed extra diopter?
Shamir Auto II digital in 1.74 only can go upto a +12 with a 350 add. close enough? with some vertex adjustment like uncle fester mentioned
A patient with a +14.00 sphere isn't going to get decent cosmetic effects anyway, why not give him a bifocal or trifocal and let him see?
I don't know if Essilor America still make it, but in the UK we can get the Varilux Omega. It's a blended lenticular prog for high plus presbyopes. Power range is:
Sphere +3.75 - +15.00
Cyl to -4.00
Adds 2.50, 3.00, 3.50, 4.00
Minimum fit height is 18mm and you need 10mm above fitting cross as well.
Might be worth giving it a go if its available?

I've had an Shamir Autograph at +13.00 combined. Actually looked decent with a good choice of frame in 1.67.
Try this bifocal, it will work well:
Carl Zeiss: Clarlet 1.5 Aphal Duopal R22
(it goes to +20 sph, 5 cyl and +3.5 Add )

I don't know if Rodenstock Manufaktur is available in India. If yes, give them a call as well.
As tsfall said, Varilux Omega Orma 1.5 will fit the bill if you insist on a PAL .

Those are the perfect lenses for this case, just do not forget their most probably in glass.
They are plastic (fortunately ), Clarlet 1.5 is CR39:
("Clarlet" is the code word used by Zeiss for Plastics).

I still order the super modular aspherics. I get them from Taylor Optical in Detroit MI. Great for high plus. The round seg is very invisible.
Remember that in Plus lenses round segs require less eye movement from distance center to near center. Progressives require the most movement of all.

The lens you are talking about is for an cataract operated patient without an implant, the addition will be 2.00-2.50. Just imagine the amount of distortion the poor patient would have to deal with on the distance as well as the reading part. A perfect pair of lenses for the garbage can.

I doubt we can get it, but the OP is in India and might have better chances (way to go U.S.A.). Another option is to grind a convex back side (saddleback), if the wearer doesn't mind the tunnel vision (1 DC boundary about 5mm in radius, with the oblique astigmatism and power error rising rapidly beyond this point (> 5 D for both 30 degrees off-axis)).

Ice-Tech in Florida can do up to a +17.00 in a Thin Ice Free-form Progressive. I have done a +10.00 myself.
What is the patients pathology? Or are they idiopathic? That will greatly effect lens choice.
Inside segment, round segment, 20mm, CR 39 or 1.56 Hivex...Quest Optics.
