I had a similar problem after my cataract surgery when one eye was a -15.00 and the other a -1.50. All will be better after the other eye is done. All they can do now is patch one eye.

And even if they can get past Anisometropia at distance. Good luck at near, going to take slab off prism, or reverse the base curves (good luck at reverseing base curves if there is a HUGE difference between the two eyes it may not be able to be done) to get rid of the double they will have at near.

Best thing to do to take care of the double is to get the IOL Sx done on the other eye ASAP. The pt. will then be VERY VERY happy.

Use some "transparent" scotch tape to "frost" the -8.00 until the next surgery.

    Aniseikonia.

Hopefully the cataracts are bilateral. Ask the client if she is more comfortable with the old glasses on or off. Trial frame the Rx as written (make sure the lens centers are in front of the pupil and as close as possible), wait twenty minutes or so and evaluate. If the diplopia is still significant, try fogging (add plus) to the fellow eye, and then do the same to the operated eye. Evaluate. Add minus to the operated eye. Evaluate. If none of the above helps, and with the doctor's approval, use a contact lens (under power to about -6.00 to balance). Over-refract for eyeglasses over the CL.

If this is a unilateral cataract, probably go right to CLs, and tell the doc that in the future, keep the postop Rx closer to the fellow eye, and if bilateral, make sure they ask their clients if they would rather have better near vision or far vision without glasses after the surgery. If they are younger and/or visually active, ask the client how much downtime they can endure between surgeries. Calculate for a postop Rx that is in the client's best interest. Do not assume that they want to reduce their reliance on corrective lenses.

Durn it, I typed the wrong term. I'm rather busy today dealing with phone outages and computer problems, and I hurried too much.

Technically both are right. Note that I misspelled aniseikonia!

    Mark,

Sounds like a bad day. Watch out for Irene.
