Getting on the medical panels

Planning to open my first optical store in NYC and I'm having hard time finding the contact numbers for the major health insurance companies such as Medicare,Empire BCBS, United healthcare, aetna, ghi, Oxford. Do any of my fellow Opticians have a number or contact info to get on those panels so I can get the applications? TIA

The Medical Panel Unit at PERAC is responsible for the scheduling of all appointments and the notification of all parties which include the applicant, retirement board, the employer and the physician(s) who will be performing the examination.
