    Confused +4.75 Add -- PAL wearer

    Hi all

    Pt Rx:-10.50 +0.50 x 45
    -12.50 +3.25 x 148
    +4.75 Add

    I have a patient that is a PAL Physio with a +4.00 Add. Last year when she came in she was supposed to have a +4.50 but the most in PAL is +4.00 so thats what we gave her. Now shes at +4.75 and her chief complaint is blurry at near.

    She will not wear a FT bifocal. Any help? Thanks

    Try to find a free form lab, I think that's going to be your only route.

    You can PM AWTECH here on optiboard as well. They have an excellent reputation and can do some amazing things.
    There are rules. Knowing those are easy. There are exceptions to the rules. Knowing those are easy. Knowing when to use them is slightly less easy. There are exceptions to the exceptions. Knowing those is a little more tricky, and know when to use those is even more so. Our industry is FULL of all of the above.

    Three Rivers Optical can do a backside blended round seg for that. In any material, including Transitions.
    Bear in mind the working distance will be short and she'll have fusion problems due to trying to converge that much. Might need to decenter a bit more for near.
    Any progressive that could be made free form will of necessity be tiny in width.
    Other than the TRO seg, Ice-Tech would be your best bet.

    Assuming the patient isn't over-minused in distance refraction, and/or has subnormal V.A., they would be better served with a pair of S.V. near glasses in addition to a weaker add for the rest of the time.

    Ice Tech can also do backside blended segs. We have done a few and they look great. Ice Tech is my go-to for complex or out of the ordinary RX's. I just sent them a -22.00+4.50 OD and -16.00 OS, the second such RX we have done.
