    Progressive Help

    I have a patient with very high cyl needing a progressive for the first time. Our lab initially suggested the Autograph, however I received a call this morning that his rx is out of range.

    His prescription is:
    OD -3.50 -5.75 x 003
    OS +2.75 -8.00 x 036
    ADD +2.25

    If anyone has any suggestions I would really appreciate it! Thanks in advance.

    Might be out of range for everybody You may also consider a slab-off.

    I think Jacqui is right. Maybe you could put them in a round seg.

    Right. Many issues here.

    Power disparity at ninety is 6.74^. VI (vertical imbalance) is likely to be more than 7^ at near, and 2.7^ at far if using a PAL with 4mm distance from the fitting point to the PRP. If the subject is binocular, it's likely there will be diplopia at near, and discomfort in general, if the VI is ignored. Segmented muultifocals and/or separates should be on the table.

    Okay a few factors on the go here. 1st, there is no slab off with round seg bifocals, so forget about that bit. Lets look at what we have.

    A request for progressives for someone with cyls so high that they are not made.


    Answer: sorry, they're not made. 2 pairs for you then.

    Job done. Why make things complicated ?

    of course, you're right, I haven't supplied a bicentric single vision for over 15 years, I'd quite forgotten they existed outside of the text books. :o

    You might be missing the obvious

    People who have always had this much imbalance in their Rx, often don't like any of our attempts at alleviating their vertical imbalance.
