If you have a PDR just set the distance that the devise will be used and use the result. You do know that the devise is adjustable for distance to be used, right?
If you have to use a PD stick once again you know that the near PD for 14-18" is usually 2 mm less than distance P.D. If the actuall working distance is to be less or more you should easily be able to come out with a workable figure by calculating (or even gestimating) the amount to be changed by the difference in the acutal working distance.

Go to Staples and buy some hole punch reinforcements or whatever they call them. These are sticky paper circles with a hole in the middle. Measure the near PD as above, then stick these thingies onto the lens where you are planning to mount the loupes. Make sure the patient can see binocularly through the holes. If not, remeasure and reposition.

You really don't want to mess up this binocular measurement and pay to remount.

Disclaimer: I don't work for Staples nor do I profit from them when you spend $2 on reinforcements.

I used to get close enough to them and have them focus on my finger held at the top of my frame and spot the lenses where their pupil was. Then I'd drill a small pilot hole (about 3 mm) and if they could see through those, then I'd go ahead and mount the scopes.
Ory, that is a great idea about the stick-on reinforcements! Ruin fewer mock lenses that way.

    Continuing Education course

I could see this information being developed into a continuing education course on low vision or specialty eyewear. This is great.

I have the patient look at the bridge of my nose and adjust the distance I'm sitting from them to about 12-13".

The hole-reinforcers is such a great idea! Thank you, Ory. :D
