  • Whats the protocol for this?

    Had a pt. come in this morning, just had cataract sx in OS and wanted me to pop her old lens out and just leave it like that. I told her, what we usually do is replace it with a plano poly. Well she got all bent out of shape and said i just wanted to sell her something. I ended up not "just" popping her lens out and explained to her that i take my patients care very seriously and would not feel like i was giving her adequate care by letting her run around with glasses that fully expose her sx eye to potential damage by sand, dust, rocks or anything unexpected, and ultimately i would be responsible!

    What do you think?

    We just pop the lens out

    Think about it, if she has the other eye done and does not need glasses, then she is not going to be wearing something over her eyes anyways.

      Make up a release form

      We offer our Post Cataract SX Pt's the same thing, only when they refuse to pay the $5.00 Plastic plano lens charge We have them sign a release form, releasing us of any responsiblity because they refused a protective lens. We buy a long white box of them fron Sola really cost effective, we are really charging for the time it takes to cut the lens, 5 min. with our slow edger.:D

        If the glasses are worn all the time and you just pop out one lens, there could be dirt trapped that can fall in the eye off the frame, Usually the ones that refuse the palno lens have had the frame since infantcey and they are really nasty. If it's a rimless frame and they refuse, please at least remove the eye liner so that does't get in there eye as well.:)

            Pop it in the ultrasonic cleaner.

            The thing is, a lot of these individuals are just holding off until the other eye is done.

              It does not take much to loosen the screen, take out the lens, and tighten the screw back up.

              Patients love it when you can help them solve their problems.

                Antibiotics...................

                Actually after a cataract operation the patient is give antibiotic drops to be taken for a month or two to protect them from any infection.

                  What in the world would you fool with a release form for. The eye is in no more danger (perhaps less) than it would be if the patient didn't wear glasses (the air is not going to shatter). Patient is correct you are just trying to make a sale. If you pop it out an patient complains of ballance or appearance, then sell them something.

                        Yes, Chris, but some people don't follow directions. I think a lens is nessasary and wouldn't let anyone go with out one.

                          I've learned to expect some protection from foreign objects entering my eye when I wear my glasses, which I do full-time. I'm not sure if I would remember to be more careful if I remove one lens but still wear the glasses.

                          Check for visual comfort with and without the lens for both near and far. In many cases removing the lens offers little benefit.

                          Consider putting a refraction on the fast track, skipping AR coatings for the temporary lens just to get the client up and running.

                          Much depends on the the Rx and other variables, but consindering the above, I'd lean towards leaving the lens in the frame.

                            Hmmm...

                            Ok, think about this.

                            Pt. comes in , just had surgery, asks you to pop th e lens out. You instead sell her a plano.

                            She trips down the steps. Her son, the attorney, sues you. You go to court.

                            -"Did you put a plano lens in her frame"

                            -"Yes"

                            _"Can you please provide for the court a copy of the prescription that instructed you to use that power?"


                            Last I checked, taking a prescription lens out and replacing it with a plano is simply just another Rx change. Are you authorized to do that?

                            (I'm not saying that I never have, but I wanted to make a point.)
