In my state there is no sales tax on low vision "aids" because they are considered a "necessary medical device used in the aid of a medical related diagnosis".
In Mississippi an independent even has to collect at retail full sales tax on a prosthetic eye as well as all optical goods.
However if the establishment is owned by a doctor(s) they pay a lower rate and pay it only on thier wholesale cost and even this is paid to the wholesale supplier (can't have those wraskely doctor's appear to be merchants even when they are selling merchandise. I don't think doctors that have out of state ocularist come into thier office and sell artificial eyes pay sales tax at all.

I pay the sales tax on frames and lenses on the wholesale price. State of California. I collect tax from the consumer on plano suns and accessories.
I'm in Arizona also and on the few occasions we would sell low vision aids we charged sales tax on all of them. You might be able to find out more information here: http://www.revenue.state.az.us/
The best thing I can suggest though is to call the state and ask them directly.
