HCFA Template

I loved our HCFA template that we had on our old computer (crashed and burned last week). We could type into it and print it out on a paper form to send in to oddball insurances. Does anyone know where I can get another one? I can't remember where it came from. We are still not completely paperless in this office and really that suits us just fine but we don't have a typewriter anymore and hand writing the HCFAs does not appeal to us at all.

Thank you. The link provided seems to be exactly what we need. Now if we can just get our data from the old XP onto the new Vista(working on it now)we will be up and running.
