Trial frame.
We use the distance to screen and figure from there.
A good start is use half the add power and go up or down from there.
Height of eye relative to screen, height of screen, would also be helpfull.
If screen is too high, you would have to use intermeadiate for distance power or give patient a PC Peeker. If screen is low enough then distance with a wide intermeadiate progressive would work. Distance with a wide progressive no matter how wonderfull the lens is, will work for say nurses with monitors higher than thier face.

Their was a thread a while ago on this subject and I dd some goofing around with calculations about the height of a desk, seated height, and viewing height. This was modifying the working distance, but it might come in handy.

Also when modifying the add power make sure to only modify the working distance in the add and no the reserve. More info here:
