    Paitents with Narrow P.D.

    :hammer: I got a client who has a p.d. of 56mm, with mild myopia and early presbyopia. She has tried the Seiko P-1SY but failed to adapt the narrow field of view on reading. Can the latest individual lenses help? Any specific frame shapes help reserve the reading portions of the PAL as much as possible during edging?

    I don't know much about the Seiko progressive but be careful of frames with sharp nasal angles.

    If the measurements are accurate, try adding some panto. That can greatly widen the reading area!

    Like Jo mentioned, watch out for frames that angle up on the nasal side, so hopefully a rectangular shape works for the pt. I recently put a patient with a 57 pd in a Unique because they will lay out the lens to maximize the reading for the given frame shape. S/A Kodak also claims they can go as low as a 13 fitting height. The patient is very happy with the glasses.

    Have you tried measuring her near PD and her distance PD seperately? Those with unusually wide or narrow eyes do not converge like the "average joe!" She's probably not reading through the full corridor.

    If she's a -1.00 with a +1.25 add, there won't be any PAL that can compete with the quality and field of vision at near when the glasses are removed.

    Zeiss, Hoya, and Rodenstock (amongst others) optimize the near inset for PD and distance Rx, aligning the corridor and near zone with more accuracy. So yes, they will help in this regard, along with adding a general sense of more refinement (less swim, easier adaptation etc.)

    However, if she can read comfortably w/o glasses then one might want to concentrate on optimizing the lens design for distance performance.
