Which CMS 1500 field defines Payee?

Does anyone know which field on the cms1500 form defines WHO the payment check is made out to. We've been having some come in our doctors name lately and i'm worried he'll get a big huge tax bill if he's 1099'd end of year. We'd been getting paid without the doctors name on the form for years, and now all of the sudden they require it and they send checks in his name (instead of our corporation). Insurance company is Fiserv Harrington Benefits. Thank you so muc

I thought box 33 defined the payee. If you are using a stamp to fill out the info make sure that you aren't stamping in the wrong spot. We had that happen; we were stamping a bit too high and one of the companies was miss-reading the first line on the address stamp.
