Could you be more specific on what is what you need?
If your lab is an OLA member they have a Progressive Identifier they can send you that has everything you need in it.

I understand the principle of copyright, but in saying that, I didnt post the document as a PDF, I posted a link to where the document it is, and has been freely available for some time. All I did to find it was google it, like I did last time I needed it

Just because someone else posts copyrighted material on their forums does not absolve me from trying to do the right thing and respect people's copyrights. It seems to me that if the OLA wanted to allow this they could have at least responded o our requests.

Laramy-K Optical has a progressive lens chart available through their website. The chart is from 5/11/2004 and therefore not exactly current, but still useful. The link is:
