I thought I new the answer to this, but I still need your help.
I have a local OD who needs a patient's records (we don't take their new Ins). We gave them all of the Doctor's Exam sheets for the past 5 years (two different ODs). They are requesting the rest of the records. The only things we have left are the job order forms for the glasses. I have no problem sending them, but are they considered part of the Medical Records? I thought the Script and the exam sheet comprised the Medical Records. If the order form was part of the record, how about the lab invoices?
