looking for eye exam (health info ) release form

i am seeking a blank form to fax to the examining doctor to release the patient's Rx to me. any help ? thank you--phil

Just send a note: Send me Rx or I will turn you in to FTC for not giving the patient a written Rx at time of exam.

Hi Phil,

When I get back to the office on Monday, i will fax you our "Hippa" Prescription Release form :D
