Gary, Hilco (perhaps others as well) have a variety of shapes, sizes and colors of full metal rim clip-ons. These can be easily manipulated to the shape of the frame or lens which you are attempting to fit. A couple pair of different shaped, nylon-jawed pliers can be used for the reshaping. if the fingers and thumbs can not make it happen.
Allow a patternless edger to trace only one of the hand made shapes and then you are ready to mount whatever lens the patient wants - including things like extra plus powers or even a bifocal.
There are several other systems out there for rimless clip-ons.
I just made a pair of polarized lenses to fit into a small pair of binoculars with only the bridge bar from the Titan frame holding them together. A serious, high-country fly fisherman. He Loves them.

A jewelry training video tape will help you in full metal rim clip mfg.
A spot welding machine, high frequency machine, polishing machine, plating equipment, and a little air brush will get you in business. A good edge is a must also.
