This keeps happening on our Alta-NX edger. looks like the groover is slipping off the lens and back on. has anyone else experienced this? We've tried a few things to fix this, like replacing the grooving tool and fiddling with the placement. but it keeps happening, seemingly at random. but always with trivex lenses.
We have an Essilor edger and the only time something like this happened was when we tried edging and didn't select the correct material. That and when our edger decided to go kaput.
It may be that your groove is a bit too far forward and is coming unstuck with the polarising layer of the lens. where they are joined, it may be weaker and grooving is not as stable there?

There may be several settings auto, front and user defined. Make sure all settings are calibrated. Looks like the default is set at front, and the calibration is off.
Does your edger have a lens measuring unit (LMU) that measures the uncut prior to edging and gives an estimate of the edge thickness and base curve? On our santinelle edger the LMU is usually what needs to be calibrated when groove position is off...it can get lens debris build up overtime that can affect the accuracy if not cleaned regularly.
I'd suggest a good cleaning and full calibration. I'd even pop the hood and make sure the internal components are clean and lubricated.
