You lay your frame on the chart and determine the curve/angle/base. Then with another chart used to verify cutout vs. powe vs decent ration, you discover if Essilor wrap lenses are possible for this frame/Rx/PD combination.

Pengukur Kelengkungan is Indonesian for "Measure the Curvature". Barry gave you the correct answer for the use of this chart.
The marks on the bottom could just be a way to confirm that the chart is printed to proper scale horizontally , it's obviously not a way to measure the A, because the marks should then start to the outside of the bridge lines (just like the angle marks at the bottom are the confirm proper printing in the vertical scale because there is no logical purpose for them otherwise) it's just a quick chart for measuring base curve, because apparently a lens clock is too hard for some people to figure out.
You should dot the lenses with a marker at the PD, then measure the curve at the PD by laying the bridge down centered on the center of the chart at the zero point. The lines at the bridge will help align the frame.
