Deepak,
I think it is Proptometer,an instrument used to asses condition called"PROPTOSIS".
This instrument measures how far forward the cornea is in relation to the orbital rim.
In your patient case file,I think it compares in relation to the other eye,i.e.left eye protuding3mm>more than right eye by using proptometer.
What I don't understand is,it is normally measured in relation to orbital rim,not the other eye.So,please find out more from his doctor and post us here.

YES, it is was proptometer measurement indeed. i called our visiting doctor to verify who says patient has bilateral grave's disease, and he recorded bulging comparasion between the eyes. Loads of thanks Optom! you have always been helpful. thanks agan.

It is also called Exophthalmometer and the measurement exophthalmometry.
