Convergence trouble??
Welcome to the ultimate myope club! I hope to remain -4.00 all my life!
Each situation requires different levels of accomodation and convergence. Contact lens accomodation and convergence is the greatest. Spectacles, next, but accomodation is assisted by increase in vertex. Removal of eyewear is similar to converging and accommodating through a +4.00, without the convergence requirement of a +4 spectacle. Accomodation loss begins at approximately age 8 and continues. Increasingly, you will note accomodative loss, first with the contacts, followed by the spectacles, but will always marvel at your ability to pull slivers, au naturale. The focal position, however, becomes more precise, and even cylinder, and dioptric difference in each eye, may cause you to close one eye. Fatigue, lighting condition(pupil size) all contribute to fusion ability. I am sure that someone from the ophthalmological/optometric panel will add to or correct me!

Start with 1 BI OU, it's enough for most of us that need an anti-fatigue lens. If it doesn't help then 2 BI OU. I use 1 in and it works for close work in the lab. Most of the Franklins we make are about 2 in.
Maybe you should have your eyes examined and tell the examiner your problem. A cover test and also distance and near phorias might show you have an uncorrected exophoria(or esophoria) and or convergence insufficiency. If CI you can do pencil push-ups as an exercise or as suggested above use 1-2 base in with a reading Rx for your close work if you are exophoric.
You are seeing double when reading and close one eye because your brain rejects the double image.
As mentioned above accomodation and convergence are linked so putting on your -4.00 Rx should help your convergence. Try putting on your Rx and put 1 prism base in before each eye, then try 2 base in before each eye and your should see a difference. Good luck
If you have no problems with near when corrected, holding material at 16" then convergence not a problem, neither are you over corrected. If uncorrected is not comfortable as it's too close for easy convergence then put your glasses on and pull out to 16" !!!!
i suppose most of the answer that cross my mind have been voice out.. don't neglect the environment issue and the kids habit.

With your 25 cm focal length, a lot of convergence is required (roughly 25^). You close one eye because staying binocular is uncomfortable, as it would be for anyone.
