  • Lifestyle Questionaire

    I am in need of help:o I need to create or borrow(depending on the help) a lifestyle questionaire. Is there a place I can go to help make one. Does anyone have a suggestion?

      fvc2020, now are you trying to ascertain information from a client so that you can determine the best eyewear solutions for their visual needs? walt might be right...so you might want to know when their birthdate is (so you know how old they are?) understanding that some vision problems/solutions are age related helps...occupational needs. many folks have mid range AND near needs, so it gives rise to fitting lenses like the Nikon Online as a second pair. so, what kinds of hobbies to folks have; some have visual safety issues, like raquetball, while others might need Rx scuba masks. do a lot of driving? polarized sun lenses work the best, golfers seems to really apprecaite transitions lenses. it helps to have an understanding of the problems that different activities present, and a thorough knowledge of what the clients visual needs are. i.e. a -4.00 myope with a 1.50 add might think they can do without their ADD in their driving lenses, not so. but you need to know why? so, who can really get by with only one set of glasses? So, if they were going to do that, what features could they combine the most effectively? just some thoughts, but lifestyle questionaires are best developed through conversations that build relationships, and in those probing questions, you get an understanding of what might best serve this unique persons lifestyle, and they will appreciate your service, beyond just the "filling the rx" deal...

            Lifestyle questioning

            Thanks for the answers.

            I have been an optician for a long time and I do lifestyle questioning w/o thinking, however, my staff is new and my doctor wants to help as well. Most classes we have been to recently recommends such a questionaire. I wanted to see what exactly was being asked so that either I can develop one myself to make patients think, my staff to remember everything, and for my practice to grow. Q and Aing a patient is great but somethings can be missed.

            Judy can you be more specific. I tried the website and couldn't find it

                I like the questionnaire on Corning's website, it hits all the major points and is simple enough not to cause your patients any distress.

                    VCA has one too. www.eyecessorize.com under the function section. they also have a needs assesment
