    Prisms

    who can help with interpretation on the prism written this way?
    OD -3.75 +2.50 x 180 add +3.00 prism 3▲BO/
    OS -2.75 +3.50 x 005 add +3.00 prism 3/2 BD

    this is the 2nd unusual use of prism I've seen in a couple weeks after 50+ yrs in the industry!

    That's the rare and elusive "CALL-THE-DAMN-MD" prism Rx. Shy and skittish, they're rarely observed outside of the exam lane, but sometimes escape as the MD is rushing out the back door to his Mercedes, for a 10:30 tee time.

    We've been trying to contact "CALL-THE-DAMN-MD" for two days. they don't answer or call back!

    Probably 3∆ BO OU and 2∆ BD OS, but yes, must be confirmed. One approach might be to talk to the office manager and explain the situation. If that fails(why?), call the patient and tell them to call the prescriber. Once you've verified the Rx, see if they're willing to let you teach the techs about prism/Rx notation and basic principles. Might be a foot in the door for a little extra work.

    That is exactly what I told the dispenser what I thought, but also I told her to still confirm with the prescriber. That's great advice to reach out to the office mgr. Thank you.

    A lot of glasses scripts from MDs are from manifest refractions done by their technicians. This could be a typo/error that slipped through and the MD didn't closely look before printing the RX? If it's a handwritten RX, then... I dunno. I wish the prescribers would put something in the notes explaining when they are prescribing something weird. I hope they get back to you with clarification.

    You're welcome. When I worked for Bensons, the offices that weren't located in or next to an MD needed to visit the local area prescribers- make an appt., usually for early AM. Most will give you 10 or 15min. Bring donuts. -Robert
