    Trapezoid Effect

    Have a 67 year old male pt. who uses SV lenses for driving and general purpose. It's remained pretty stable for ~4 years at:

    -1.25 -0.75 064
    -1.50 -0.50 088


    Earlier this year he came in for refraction, which is now:


    -1.50 -1.00 080
    -1.50 -0.25 120

    The pt. kept the same frame, got poly with standard AR, which is exactly what he's worn for four years. Now, though, he keeps coming in complaining that the OD "bends" into a trapezoid as it nears the temporal side of the lens. We've tried rerefracting him (negligible difference in axis), remaking the lenses, and ordering a new, identical frame, but he just keeps returning with the same issue. The last set of lenses we had our lab manager and two separate QC experts examine, and all of them found them to be perfect. Anyone have any ideas?

    Maybe a vascular problem. Just make sure it's not the eyeglasses (the visual field defect occurs with the old lenses, or with the lenses removed from the frame) before you recommend they call an ophthalmologist today.

    I'm thinking like drk that this a is big change in axis after four years of stability at his age. I would want to take it back a bit too.

    Thanks for the replies, folks!

    Had the pt. come back in and did some experimentation... Turns out he does have a hypersensitivity to the degree of axis. Used some trial frames to test out different RXes and axes and discovered that even moving the axis 3-4 degrees caused the effect. Gonna keep the sphere and cyl and switch the axis back and see what happens.
