You need a HIPAA form for each patient.

As an Optician running an optical store with an independent OD, who obtains the hippa form for each patient he sees do I need one for each spectacle order?

You really need to read up on your legal requirements. The link I provided you will give you everything you need to know.

But, basically, for every patient the store sees, you need to have a HIPAA form on file. This is required because of the separation between the OD and the store.

Mike is saying what you should do. If you do not comply here is a quick snip of the complaint process-

I highly doubt the Department of Justice is going to prosecute an optician so you risk at most a fine . More likely a requirement to comply.

    Your choice.
