I may be able to do it, but usually once we hit about -20 the equipment starts to complain.

I would probably forget a PAL solution. If the client is a good candidate then contact lenses and readers would give the best visual acuity. If not or as a back up solution then a myio- disc in hi index glass with AR and a slight tint from a quality lab in distance single vision would be an alternative. Frame would have to be an MED fit. For reading a separate pair or for temporary reading just slide the distance rx further down the nose.
I'ma thinking flat top, fit high.
Not to add insult to injury, but them's already ugly, so I doubt a seg line could mess it up more.

Thanks for your suggestions guys, but this is a vain little old lady with some knowledge who knows what she wants and will not settle for anything else. Eyes become too dry with extended CL wear. Old script has a total power of -20 and it is working for her, probably because she has willed it to be so. The doc is going to try to tweak the new script down a bit this afternoon. PD is 26/28 (please save the comments about decentration) with 41 in a three piece. So all I need to know is who can make this. I've got one really good lead already. Any more ideas?

Hey I made my wife a -10, three piece silhouett compression mount in 1.74 progressive. Its awesome. So many people spaz out when you start asking 3 piece anything. But if you pay attention to size, shape, material, decentration, type of person and their expiation's and your ability's. You would be surprised at what you can do with today's technology.
A minus 20 though. Oh my. But if she presents wearing a -20 three piece and wants a -20 something 3 piece it obvious that it can be done. Just do it. Or send her to someone who can and will.
...and a 26mm RT mono IPD for about 5mm to 7mm of decentration. A lab could charge 3x usual and customary and still lose big money.
Zeiss might be able to surface the progressive optics and astigmatism on the front with a free-form generator, then finish on the back with an old sphere generator for the high minus component.
Put SV 1.67 or 1.74 on the table, one pair each for distance and near.
If separates are not practical, maybe a minus lenticular (30-35mm bowl) segmented, if anyone can still do it.

If going the 3 piece route, I would do 1mm holes 3mm deep without compression fittings.
