From the other threads I've seen on the topic (and you're bound to get a decent response to this, BTW), it often helps to look at the reasons for the 're-dos', because if they're all for the same reason (slightly unlikely, but possible), you may well be able to improve the re-do rate.
I agree. You need to break down that percentage into its components to see where the training needs to be focused.
Is that a 9% for an internal breakage percentage? Or does this include things like non-adapts?
If your lab is a member of The Vision Council's Optical Laboratory Division (ex OLA), you should participate in the Shared Lab Stats program. This can be a useful benchmarking tool for seeing how your lab stacks up against other labs.
