Franklin bifocal.
Not enough add power for a decentered FT45. If it's an adult, they could remove the eyeglasses for prolonged near tasks. SV readers, prism segs, free-form generated multifocals (if possible), should be on the table, in addition to the aforementioned Franklin.

Great opportunity to recommend two pairs. Franklin bifocals would work too but the are $$$
You could first try the patient with press on fresnel prisms full size , then cut them in half to cover the distance only. That will tell you a story.
Who is requesting the distance only prism, the prescriber or the patient ?
What would your license allow you to do with that prescribers Rx if the patient requested SV reading glasses only ? Is the Rx written to allow you to make readers without prism ?
