Only if a frame (in warranty) breaks, causing the lens to break (eyewire screw loss excluded).
We do not charge an up front fee for any warranty. The warranty is included with the purchase. What we do charge for is a lens or frame co-pay for each incident. One year only. Different co-pays for various lens.
Do yourself a favor, never offer a policy of any sort that doesn't include the return of the old lens.
Years ago, in the PMMA days, contact lenses were often "insured" against loss. A company called RLI issued most of those policys. When frequent-replacement lenses became the norm, they tried eyeglass-loss insurance. They didn't last long. It turns out that the loss rate approached 100%. Let the sharpies who inhabit our present world know that they can get a replacement or extra set just by asking, and they will use you up.
Not sure what you mean but no, we do not charge for a warranty. I honor manufacturer warranties and guarantees but I'm clear about abuse and carelessness- it will not be my problem if a patient cannot take care of their investment.
