Do NOT be afraid doc. THAT is the very problem that these managed care companies manufacture, perpetuate, and prey on. Do what you do better than everyone else. If you have to rely on some third party company to fill your chair - as a business, you're doing something VERY wrong.
What good is it to keep a patient "not turn them away" if you aren't making any money on the time and effort you put into the encounter?????

I am choosing to do it. If you treat people right, you DO get referrals, sometimes even the immediate relatives that pay cash!, and when these kids grow up and get different insurance, they are already your patients. If you look at what the one transaction does for your business, you may be myopic.

By frightfully slow are you, if you are referring to your business, you may want to create a time management chart. How much time do you spend with your patients per exam divided by VSP pay out. How much are you going to make on a managed care frame provided by said HMO for which you receive a small dispensing. How much are you paying your staff, how much time is that staff member spending with these patients. How problematic is the product in question, children are notoriously hard on their glasses. You should figure on a large percentage of these patients coming back for repairs and adjustments and the costs that you will be incurring for this type of work. What about your inventory, if you sell frames what percentage of time will be taken away from your staff when they could be selling from your inventory. These are all issues many of us are forced to deal with and when faced with changes whether due to the economy or HMO I always look at the bottom line, your dollar.

My guess is if there aren't enough providers on the private practice side, they would likely push them to their retail affiliate partners or local clinics. That would not be good. Careful what you wish for.
You may be right, and I hate to be argumentative, but I don't see these kids growing up with an appreciation for quality good and services, much less actually paying for them. I feel like they are going to become young adults who expect "free" stuff and will choke on the cost of a custom made medical device made by a professional. I'm not too terribly optimistic these day... :(

For those like me who aren't in one of the O&P zones here's the catalog https://www-acpt.vsp.com/dms-inline/...er-Catalog.pdf and they really aren't that bad. Though I think there's too many plastics for a children's line and there's a 56-16-145 plastic (OP4004) , I know kid's heads are getting bigger but I can't remember ever selling (or wanting) a 56 eye with that small of a bridge.
We received ours, I opened the box, replaced them all with a nasty Logo frame that I had 17 of (same frame/same size/color). I repacked them, taped the box up, and put them under the incoming mail. You should have seen the look on the staffs' faces!! Priceless. At their suggestion, we might just leave the nasty frames in there...

We just called VSP and were told that our practice would not be receiving the Otis & Piper frame kit before the first of the year, but that we WOULD be receiving a letter from VSP along with measurement aids and a website address where we can view the frames. When I asked the VSP representative when we could expect our kit to arrive, he said he was not aware that we would ever be receiving the kit, but we could still accurately fit children based on the measurement aids that VSP would be sending us. WHAT??????!!!!!!! How is that patient care?? How is that helping the independent eye clinics?????
Measurement aids? What do they mean by that?
Well I remember the measurement aid they sent for the Unity lens, it was a plastic card with the lines for wrap, panto, vertex with a PD stick stuck in a slot they made in the eye to show you where to start the measurement for vertex (I might still even have the PD stick).
Maybe they'll be a fixation tool, I don't know what they could mean by measurement.
I'm sure that if the O&P frames are as good quality as the Sight for Student frames we will have no problems with them at all.
