    Guidance request on a plus prescription

    I have a client that is a plus prescription. She was given eyewear by an optometrist with a higher plus so she could avoid going into progressives for a few years. So she never had perfect up close or distance.
    Her old rx was:
    OD: +3.50 -2.00 082
    OS: +4.00 -2.75 096

    Her new RX (from a different place) is as follows:
    OD +2.75 -1.75
    OS +3.00 -2.75

    ADD +2.00

    When fitting her in Contact lenses it takes her awhile to bring the rx into focus (both of them above). Her vision is in and out ou20/20 od 20/20- os20/25. I filled her distance rx into the trial frame she is able to see but it takes her a few seconds before its clear. (old RX not clear but still manages to bring it into focus). Whats going on? Seems like her best correction it still requires her to bring it in focus.

    I am curious to why this is happening. Any thoughts?

    Thanks. We will be making her progressives for her so we shall see how it goes. Yes this did help. I did research it love GOOGLE. Thanks Robert

    Sound like a slow accommodative reflex to me.

    HyperopiC astigmats of this nature commonly have some refractive amblyopia. Their refractions are often unstable. They often go without glasses because their spherical equivalent is almost plano.

    You don't mention her age, but it is generally unwise to reduce someone's plus if they are near presbyopia, and have been successfully wearing the higher plus. Pushing her into a higher add due to reduced plus at distance is also not the best idea. A toric soft cl will further compromise best VA.

    These are tough cases.
