    She is wearing Fresnel prism. I hated that stuff, but who ever installed hers did it right. Who is the DC or NY optician that works with her? What frame is she wearing?

    By "right", I guess you mean recessed from the edge of the lens: but this is pretty rough looking, too far from the edge, and apparently scissor-cut. It could be neater.

    Ms. Clinton has a VI Cranial Nerve dysfunction or even palsy secondary probably to trauma. Since this causes loss of abduction in the left eye, she needed a large enough frame to hold the Frensel lens. A smaller frame would not work because it is possible that her gaze to the left would go outside the frame.

    I believe that she turns her head to the left now to avoid left field diplopia. She should expect a recovery period of 9-12 months and if not, then a more permanent solution may be needed.

    You are right, it looked better in the other pic I had, no edges showing. At least its clear.

    My thoughts exactly. She can definitely do something bold, but those just look chunky and uncomfortable.

    How would you or one of you other top notch benchmen do it?

    I'm thinking create a pattern and use a razor?

    Thanks npdr for the explanation. I was hoping a doc would speculate just what you did!

    It's been a while, but I always placed the prism on the lens, cut out the rough shape with an exacto knife, and then finished it on finishing wheel with very light pressure until i had the exact size, this would give an edge to edge finish and looked as well as a Fresnell prism could look.

    Here's my thought on the subject. The Fresnel was probably applied by her MD or her MD's tech. Having worked in a practice that specialized in adult strabismus, the MD often applied the prism using whatever tools were handy. The MD also saved the prisms and reused them, since they are supposed to be a temporary fix. I tried rather unsuccessfully to change those habits, so some really nice eyewear often looked pretty sketchy.

    Why the heck didn't they just grind the lenses with prism? Money an issue?

    Since it is possibly a temporary test or fix. You wouldn't grind it. You would do as they did, press-on.

    The MD doesn't worry about that. The MD is concerned about correcting a visual issue. Hillary is probably more concerned with completing her term as Secretary of State than what her glasses look like for a few moments on TV. I do know one of her Opticians and I can tell you that is not her work.

    Post trauma diplopia recovery is uneven and unpredictable. Fresnel is perfect because her power will change in one month, and it make take 6-12 to fully recover with different powers at each phase.. Because she might have 4-5 Prism changes during that time, its far easier to change and test with Fresnel.

    Even when I went permanent, I often prefered to start out with Fresnel because it allowed for some adjustment before finalizing the RX. To RX prism, it really takes leaving a pt in a trial pair for an hour or more, not something most MDs will do.

    Use a scalpel to cut the fresnel with a sawing motion. As Randle suggests above, press the decal onto the lens, then saw at a 45 degree angle to the back side (thereby beveling back the fresnel so the frame groove doesn't lift it). Make the cut flatter to the minus side than the angle of the pin bevel on the lens. Use a new, sharp disposable scalpel so you don't have to press hard enough to flake off the back edge of the bevel. One step, very quick, very neat, perfect shape.

    Grinding is really only an option up to 6 or 8 diopters, then it would really look ridiculous.

    We do a bit of strabismic work and when we have to cut one of these babies, we orient it on the lens properly, then trace the exact lens shape with a sharpie and cut it with a good scissor. You really can't make a pattern because the base curve of the lens will alter the final size and shape. You should keep the size of the fresnel 1 mm from the safety bevel on the lens to prevent peeling. But there is no excuse for it being several mms from the edge like in the photos.

    BTW, I'm not buying the story that she got dizzy from the flu and exhaustion, fell and bumped her head, causing diplopia and a blood clot. I'm speculating she had a mild CVA which caused the diplopia, and a fall from loss of consciousness. And if she fell and hit her head that was after the fact. But I shouldn't be speculating...
