It's a Hoya lens. Not sure which one exactly but I think one of the ID family, which is a great lens if you haven't dispensed it before. One of the best designs in the world.

Would you like any more detail?

It is the ID Mystyle, the most bespoke version of the design. 12F means 1.25 add all on the front surface. 43 means 1.6 index material.

There should also be an M followed by either a B, C or an O. This indicates whether the design is 'Balance', 'Clear' or 'Open'.

12F means 1.25 add all on the front surface.
12F means that the 1.25 add power is measured at the front, not that it is produced on the front. ID Mystyle is a dual surface design that has elements on both front and back.
